Once you have received your welcome email for therapy portal you can click on documents and you will see a list of them.
Please fill out each form electronically and sign including but not limited to the following: Client information form, Client history form, Client insurance form, Consent for Services, Payment Authorization form, Notice of Privacy Practices.
